Job Summary
Provide direct supervision, coaching, and oversight to a team of Case Managers while monitoring service quality, ensuring contractual compliance, and managing staff performance. Conduct regular individual supervision, team meetings, and performance evaluations to support professional development and maintain consistency in case management practices. Review assessments, housing plans, and documentation for accuracy; facilitate case conferences and interdisciplinary meetings to address barriers to housing stability. Ensure all client information is entered into HMIS accurately and generate reports to monitor program outcomes and contract compliance. Serve as the primary liaison with Montgomery County's Services to End and Prevent Homelessness and other community partners to coordinate seamless service delivery and advocate for clients.
Required Qualifications
- Master's degree in Social Work, Human Services, Psychology, Counseling, or a related field
- Three to five years of progressively responsible case management experience
- Experience working with individuals experiencing homelessness, behavioral health challenges, substance use disorders, or other vulnerable populations
- Must occasionally lift and/or move up to 25 pounds
- Specific vision abilities including close vision, distance vision, and the ability to adjust focus
- Regularly required to talk or hear
- Frequently required to use hands or fingers, handle or feel objects, tools, or controls
- Occasionally required to stand; walk; sit; and reach with hands and arms
- Drug free workplace compliance
Desired Qualifications
- Clinical licensure
- Previous supervisory or lead experience
- Experience supervising case management teams serving individuals experiencing homelessness
- Working knowledge of Montgomery County's homeless services system
- Experience utilizing HMIS and data-driven performance management
- Knowledge of HUD, CoC, ESG, RRH, PSH, and emergency shelter regulations
- Experience collaborating with SEPH and Continuum of Care partners
- Demonstrated success improving program outcomes through coaching, quality assurance, and performance management
