Job Summary
Conduct comprehensive, long-term case management services for clients in an established life management program, developing program plans and goals while evaluating progress through mentoring and counseling sessions. Act as an advocate to acquire services enabling functional coping, establish unique three-month program goals based on client capabilities and history, and consistently counsel on modifying negative behaviors. Manage caseloads by outsourcing to external resources, preparing case presentations for coalitions and funding bodies, and processing financial assistance for rent, utilities, and food vouchers. Locate temporary housing, conduct home visitations and inspections, and refer clients to appropriate Corps programs or agencies. Maintain accurate records, enforce disciplinary actions, and distribute prepared food bags to clients within a shelter environment.
Required Qualifications
- Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field
- Valid State Drivers License
- Three years progressively responsible experience providing direct case management social services including accessing clients' needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources
- any equivalent combination of training and experience which provides the required knowledge, skills, and abilities
- Ability to read, write, and communicate the English language effectively
- Ability to communicate clearly and effectively both orally and in writing
- Ability to think clearly and quickly in order to maintain control of client caseload
- Ability to effectively and efficiently work on multiple cases at the same time without becoming frustrated or disorganized
- Ability to meet attendance requirements
- Schedule: Tuesday thru Saturday
