Job Summary
Case Management Specialist responsible for conducting interviews to determine eligibility for program enrollment, establishing and evaluating long-term program goals, counseling clients to modify negative behaviors, coordinating external counseling resources, and maintaining accurate records within an established life management program. The role includes advocating for clients to access services, assisting with income supports and benefits, and ensuring compliance with funding requirements. Requires a Bachelor’s degree in social work, behavioral science, or related field and three years of direct case management experience. Location: Albany, GA; compensation approximately $21.50 hourly (annualized to about $44,720).
Required Qualifications
- Bachelor’s degree in social work, behavioral science, or related field
- Three (3) years progressively responsible experience providing direct case management, social services including accessing clients’ needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources
- Valid State Driver’s License
- Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities
