Job Summary
Evaluate and treat admitted individuals with brain or spinal cord injuries by gathering data from records, interviews, and team dialogue. Develop and implement treatment plans according to physician orders, including establishing goals and re-evaluating progress. Document individual progress through initial evaluations, progress reports, discharge summaries, and billing information. Counsel individuals and family members in their homes or appropriate settings to facilitate adjustment and understanding of the recovery process. Attend team conferences to report on individual status and collaboratively assist in identifying realistic goals. Communicate effectively with internal case managers while maintaining a valid driver's license and access to a registered vehicle.
Required Qualifications
- Master's Degree in Social Work from an accredited school of social work
- Membership in NASW
- related clinical experience in a rehabilitation setting with a focus on traumatic brain injury and spinal cord injury
- Valid driver's license in good standing
