Job Summary
Coordinate Individual Service Plans with the Social Worker while providing continuous outreach to young adult participants aged 18-21. Identify high-risk individuals, conduct initial engagement, and maintain trust to facilitate connections with local resources, Cure Violence providers, and community activities. Monitor client progress through scheduled telephone contacts and office visits, document outcomes, and recommend case closures based on goal achievement or eligibility status. Assist in data collection, organize Peer Support Leaders, and escort clients to school as needed. Provide weekly reporting and conduct periodic reassessments to ensure program compliance and support stability for families and neighbors in Van Dyke.
Required Qualifications
- Bachelor's degree (B. A. or B. S.) and one year of applicable experience
- Associate's degree (A.S) and two years of applicable experience
- High School Diploma or HSE and three years of applicable experience
- Relevant experience as described above, preferably in a mid-sized (250+ employees) organization
- Good written & verbal communication
- Computer literacy in Microsoft Office Suite
- Strong knowledge of the Canarsie community
- Flexibility with working a flexible schedule including morning, evening and weekends as needed, and be prepared for various outdoor weather conditions during their workday
