Job Summary
Set clinical and operational direction for the Case Management program by coaching supervisors, aligning practice standards, and tracking quality outcomes. Conduct daily pulse-checks on metrics and overnight updates, then lead huddles with supervisors and quick consults with multidisciplinary partners to ensure smooth communication across clinic and community sites. Review dashboards and documentation patterns to spot trends and tee up practical improvements, while problem-solving barriers to care and keeping workflows steady during change. Maintain audit readiness and mission alignment through steady collaboration and systems-thinking.
Required Qualifications
- Master's in Social Work, Mental Health Counseling, or related field
- Active Utah licensure (LCSW, CSW, ACMHC, or CMHC)
- 5+ years medical and/or behavioral health case management (or related)
- 5+ years leadership experience; able to supervise supervisors/managers
- Experience with high-acuity, medically complex populations and homelessness/behavioral health/SUD
- EHR proficiency; strong communication, conflict resolution, and change leadership
- Trauma-informed, harm-reduction, health equity commitment
- Valid UT driver's license + insurability
- Must be able to pass a background check, including fingerprints
