Job Summary
Case Management Coordinator (Field) conducts field-based care coordination to support healthcare outcomes for members, providing advocacy, collaboration, coordination, support, and education through care management tools and resources. Requires 80-90% regional in-state travel with anticipated travel to Louisville for meetings and training. Must reside in The Lakes Region counties in Kentucky. Role focuses on serving children and families with after-hours flexibility, and involves working within behavioral health, foster care, and crisis intervention contexts to support SKY populations in Kentucky.
Required Qualifications
- Minimum 6 months experience in Foster Care and juvenile justice systems
- Adoption Assistance
- delivery of Behavioral Health Services
- Trauma-informed Care
- ACEs
- Crisis Intervention services or evidence-based practices applicable to the Kentucky SKY populations
- Minimum 1 year of experience in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling
- Minimum 1 year of experience in MS Office Suite applications, including Word and Excel
- Bachelor's degree or non-licensed master level clinician in behavioral health or human services field
