Job Summary
Conduct comprehensive evaluations of member needs and eligibility using care management tools to recommend case resolution approaches. Coordinate assigned care plan activities, monitor progress, and facilitate appropriate healthcare outcomes through appointment scheduling and benefit access assistance. Escalate quality of care issues, consult with medical directors, and present cases at multidisciplinary conferences to achieve optimal outcomes. Utilize negotiation and motivational interviewing skills to empower members for independent lifestyle choices and engage in peer mentoring. Maintain documentation in compliance with regulatory guidelines while traveling 50-75% of the time in Mason, Cass, Scott, and Brown counties. Requires a Bachelor's degree in Behavioral Health or Human Services with 2+ years of relevant experience.
Required Qualifications
- 2+ years experience in Behavioral Health, Social Services or appropriate related field equivalent to program focus
- Must be willing and able to travel 50-75% of the time in Mason, Cass, Scott and Brown counties, Illinois
- Reliable transportation
- Must have 2+ years of experience of electronic documentation experience and Microsoft Office applications
- Bachelor's degree or non-licensed master level clinician required
