Case Management Coordinator

Job Summary
Case Management Coordinator responsible for engaging individuals in a welcoming, hopeful, and empathic manner, screening for co-occurring disorders, and assessing needs to develop stage-wise, strength-based, person-centered plans. Completes referral forms and necessary documentation to obtain community-based services, coordinates collaboration with SA/MH partners, advocates for supports, and develops connections to community resources to address identified needs.
Required Qualifications
- Must be a QMHP in accordance with Medicaid Provider Manual Guidelines
- Bachelor’s degree in a human services field OR equivalent experience
- specialized training/fieldwork/internships with mental health services experience OR one year of experience in treating or working with a person who has mental illness
