Job Summary
The Case Management Assistant (CMA) is a key member of the Integrated Care Management team, responsible for conducting outreach and assessing patients during care transitions, addressing gaps in care, and providing education on health maintenance. This role requires knowledge of medical terminology, current computer skills, and effective communication abilities. The CMA supports value-based care initiatives and must have an AA degree along with 3-5 years of clinical experience in a healthcare setting, CPR certification, and relevant certifications are preferred. The position is part-time with a pay range of $19.16 to $27.77 per hour based on experience and skills.
Required Qualifications
- AA degree with at least 3-5 years of clinical work experience in a healthcare setting
- Current CPR certification
- Knowledge of medical terminology
- Current computer skills including Microsoft Office Products (Word, Excel, Outlook, Teams)
- Effective interpersonal and communication skills
- Excellent time management skills
- Ability to collaborate with a multidisciplinary care team
