Case Management Assistant II

Coordinates and implements the transition of care (TOC) / Discharge (DC) plan for ambulatory patients, prioritizing and coordinating the plan across the care continuum through collaboration with care providers, patients, families and external vendors to ensure timely discharge. Requires HS diploma or equivalent and at least 1 year of experience in utilization management and discharge planning (preferred). Strong oral and written communication, interpersonal, and time management skills, ability to work in a fast-paced environment, and proficiency with PC/word processing tools. Per diem/casual role with dayshift, variable hours, and rotating weekends at Sutter Health in San Francisco, CA.
