Case Management Assistant (Administration)

Job Summary
Administrative support to the Case Management team by coordinating patient dispositions and handling referrals to post-acute service providers. Duties include documenting in the Trinity-approved EMR, managing communications via telephone, voicemail, fax, and mail, following up with insurance payers and post-acute providers, issuing Medicare Important Message Letters, and maintaining knowledge of applicable laws and Trinity Health policies. Requires initiative, independent decision-making, attention to detail, integrity, and confidentiality, along with a working understanding of admissions, management information systems, medical records, insurance, patient billing, medical terminology, and payer contract terms. Proficient in EPIC, Microsoft Office suite, and related tools; ability to establish collaborative relationships and perform other duties as assigned.
Required Qualifications
- Education: High School graduate or Associate degree in Medical Technology, Business or Healthcare (as stated)
- Experience: 1-2 years in health-related field preferred
- Skills: Strong organization, prioritization, interpersonal and telephonic/electronic communication abilities
- Computer: Proficiency with EPIC, Microsoft Word, Teams, Outlook, Excel, PowerPoint
