Case Coordinator

Job Summary
Coordinate assessments, treatment planning, group therapy, individual therapy, family therapy, and aftercare planning for a caseload of Psychiatric Residential Treatment Facility residents. Conduct case planning conferences and ensure proper discharge planning with full documentation. Provide 24-hour, on-call support including crisis intervention when required. Maintain close relationships with public and private agencies while ensuring all activities comply with accreditation and regulatory standards. Demonstrate skills in separation, loss issues, family systems, conflict resolution, and crisis intervention techniques. Travel to attend meetings, training, or conduct business off site.
Required Qualifications
- Bachelor's degree in Human Services field
- Must have a minimum of 2 years experience working with emotionally disturbed children in a residential setting
- Must have a valid driver's license
- Must be able to travel in order to attend meetings, training, or conduct business off site
Desired Qualifications
- Master's degree in Human Services field preferred
