Job Summary
Develop and implement personalized training plans for youth, families, and adults across homes, schools, and community settings. Provide engaging one-on-one and group sessions focused on daily living skills, social skills, and parenting education while applying evidence-based practices. Offer ongoing mentorship, help clients apply new skills in real-world situations, and collaborate closely with families and service providers to ensure holistic care. Participate in team meetings and case conferences to monitor progress, maintain accurate documentation, and connect clients with community resources. Requires a Bachelor's degree in psychology, Social Work, Education, or Rehabilitation Services; relevant certifications such as CRC are a plus. Valid driver's license and reliable transportation are mandatory for visiting clients.
