Job Summary
Case Coordinator responsible for collaborating with Veterans and team members to identify and secure affordable housing, developing Housing Stability Plans, increasing Veteran income for housing stability, maintaining detailed case notes (HMIS/Clarity), providing ongoing referrals and resources, coordinating transportation to appointments, and completing required trainings. Requires a Bachelor's degree (or two years of equivalent experience in work with Veterans or homelessness), First-Aid/CPR certification (or within 45 days of hire), and a valid driver's license with an insured vehicle. Travel within California counties and quarterly statewide travel expected. This is a full-time position with a focus on helping Veterans secure permanent housing and navigate services within a nonprofit organization in California.
Required Qualifications
- 18 years of age or older
- Bachelor’s degree or two years of equivalent experience and/or educational equivalent in working with veterans and/or with individuals who have experienced homelessness and/or are at imminent risk of homelessness
- Certification in an approved First-Aid and CPR class; or successfully complete an approved First-Aid and CPR class within 45 days of the date of hire
- Must successfully complete Adjoin’s training programs as assigned
- Insured, registered vehicle in safe working condition, and proof of current driver’s license
- Ability to travel within and throughout the counties in which services are provided
- Ability to travel in and throughout the state on a quarterly basis to attend meetings, trainings, etc.
- Successful completion of pre-employment drug screen and background check
- Acceptable DMV record
