Job Summary
Case Associate role focused on connecting families to concrete resources and prosocial activities. Functions as a Case Planner, supplying appropriate interventions to ensure client care, developing action plans, identifying and utilizing existing resources, and establishing new resources for client care. Responsibilities include routine community contacts (e.g., school and medical reports), home visits to evaluate living conditions, explaining agency functions to clients, guiding independent living skills, running groups under supervision, evaluating clients for individualized treatment programs (including child safety), referring clients to internal/external/community resources, performing individual/group counseling (short- or long-term as needed), conducting community outreach, documenting progress notes and risk assessments, and ensuring access to necessary services. Core competencies emphasize engaging families and teens from diverse backgrounds. Educational/training requirement: Bachelors Degree with experience in community or social groups, or at least five years as a case assistant in social services. TravelRequired: true.
