Job Summary
Conduct culturally competent case management and advocacy for families in the FFT-CW Program, assisting therapists with collecting educational and medical data while linking clients to community resources for housing, public assistance, and mental health services. Perform concrete case management tasks including rent arrears assistance, utility support, and furniture aid, while maintaining case records on PROMIS and CONNX systems. Monitor school attendance, obtain medical records, and conduct "Safe Sleep" and risk assessments for children. Accompany families to entitlement appointments, assist with Family Team Conferences, and make referrals to agencies for pro-social activities and vocational programs. Complete required child welfare documentation and screening forms, answer telephone calls, and cover the front desk as needed.
Required Qualifications
- Associate degree
- Minimum one year of related experience working with children, adolescents, and/or families
- Experience preferably in a child welfare, juvenile justice, or mental health settings
- Ability and willingness to work with families and teens in the home, school, and other appropriate settings in the community
- Ability to work flexible hours including evenings and some weekends as needed
- Ability to work independently, as well as contribute to a team
- Very good interpersonal/communication skills
- Ability to write well
- Ability to complete documentation in a timely manner
- Computer literate
- Ability to attend all trainings necessary to perform the above tasks
Desired Qualifications
- Bachelor's degree in social work (BSW)
- BA in a related field
- Bilingual in Spanish/English speaking
