Carmel, NY - Discharge Planner (CASAC or LMHC)

Conduct comprehensive discharge planning for patients, overseeing difficult placements and ensuring rehab appointments with primary care providers. Maintain ongoing communication with clinical teams, families, legal entities, and referral agencies while providing psycho-education, life skills, and relapse prevention support. Follow up on missed appointments, document patient progress and interventions, and coordinate linkage agreements for aftercare services. Participate in daily treatment planning, quality improvement activities, and emergency procedures, while supervising interns and volunteers as needed. Maintain professional competence through continuing education and supervise interns and volunteers at the director's discretion.
