Job Summary
Conduct in-home personal care, including bathing, toileting, mobility assistance, and dementia support, while maintaining client dignity and safety. Provide companionship, meal preparation, light housekeeping, and medication reminders under approved care plans. Perform transfers using gait belts or lifts, monitor blood glucose, and document all services in real time. Complete safety checks and nighttime routines for overnight shifts, ensuring full alertness and immediate reporting of any behavioral or physical changes. Maintain a clean environment, manage transportation to appointments, and adhere to strict attendance and confidentiality protocols.
