Job Summary
Caregiver provides in-home personal care and comfort for clients per the plan of care under supervision, including med reminders, meal preparation, light housekeeping, ADLs, and companionship. The role operates in clients' residences with a flexible Part-Time schedule (Monday–Friday, no holidays). Paid orientation and ongoing training, with benefits including 401k, Flexible Spending Accounts, Tuition Reimbursement, Employee Assistance Program, and referral programs. Transition At Home emphasizes compassionate, patient-centered care and a supportive team culture; expected to collaborate with the NM Manager and coordinate care as part of a home health team.
Required Qualifications
- Must be a minimum of 17 years of age
- One year of Caregiving experience preferred
- A valid Wisconsin Driver's License and Car Insurance
