Job Summary
Care Manager/Designated Care Manager responsible for providing high-quality care to a consistent group of residents in Sunrise Senior Living's assisted living/long-term care and reminiscence neighborhoods. Duties include documenting through ISPs, developing and updating individualized service plans, monitoring residents’ conditions and needs, coordinating with families and supervisors, maintaining safety and cleanliness, assisting with activities of daily living, and promoting resident dignity and engagement through Life Skills and reminiscence activities. Requires strong communication, empathy, safety awareness, infection control, and adherence to Sunrise standards and regulatory requirements. Must have a High School diploma/GED, CNA or HHA certification, CPR/First Aid as required, and be at least 18 years old; prior experience with seniors preferred. Required Qualifications
- High School diploma/GED accepted
- CNA or HHA Certification
- CPR Certification and First Aid as required by state/provincial regulations
- Must be at least 18 years of age
- Previous experience working with seniors preferred
