Job Summary
Provide personal care and support services to clients in their homes or community settings, assisting with daily living activities as outlined in care plans. Maintain a safe, respectful environment while accurately documenting all services delivered using approved systems. Report incidents, concerns, or changes in client condition immediately and communicate promptly regarding schedule changes or emergencies. Arrive on time for all scheduled visits and complete assigned tasks while maintaining professional boundaries and confidentiality with clients, families, and team members. This field-based role involves prolonged standing, walking, and lifting, with overtime eligible for variable shifts including days, evenings, and weekends.
Required Qualifications
- Minimum 1 year of caregiving or related experience
- Ability to follow care plans and instructions
- Reliable transportation within service area
- Ability to pass background check and required screenings
