Job Summary
Provide personal care and delegated health-related services to residents per individualized care plans; assist with activities of daily living (dressing, grooming, bathing, eating, toileting, ambulation, transferring), AM and HS cares (including oral care), incontinence care, assistance with assistive devices, glasses, and hearing aids; administer medications as delegated by the RN; observe and document changes in residents' physical, cognitive, and emotional condition; promote independence and self-help skills; collaborate with Clinical Director or RN and other team members; perform scheduled cleaning tasks; lead and assist resident activities; respond to safety hazards and call lights; perform other duties as assigned; shift options include Part Time Afternoon/PM or Overnight with every other weekend rotation.
Required Qualifications
- Must be 16 years of age or older
- Ability to communicate effectively both verbal and written
- Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident’s Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations
