Job Summary
Caregiver/QMAP role providing direct support and rehabilitation services in community settings. Responsibilities include assisting with socialization, personal care, daily living activities, transportation, and community orientation; accurately documenting progress and billing information; managing individuals' assets and finances when assigned; reporting incidents and ensuring safety. May accompany individuals to medical appointments, administer medications when assigned, monitor health, and follow health care directives. Maintain confidentiality and professional relationships with individuals, families, guardians, and case managers. Required to have QMAP certification, valid driver’s license, and high school diploma or equivalent; training, orientation, and ongoing staff meetings are provided; shifts include day or night. Demonstrates teamwork, safety awareness, and adherence to policies, with duties including transportation, housekeeping, maintenance tasks, and supporting health-related needs. Salary range is $19-$22 per hour, paid hourly.
Required Qualifications
- QMAP Certification Required
- High school diploma or equivalent
- Valid driver’s license
- Six months of experience in human services preferred
- Blood Borne Pathogen Certification
- Certified training completed in mandated timeframes
