Caregiver

Conduct all duties as assigned by the client's Care Plan, including assisting with activities of daily living such as bathing, grooming, dressing, toileting, oral hygiene, ambulation, and feeding. Prepare meals to meet nutritional needs, perform light housekeeping like sanitizing rooms and laundry, and support safe transfers and ambulation. Remind clients to take self-administered medications, encourage light exercise per the Care Plan, and observe/report changes in mental, physical, or emotional condition to Hands from the Heart. Maintain effective communication with clients, families, and co-workers while completing required documentation, using equipment safely, and upholding confidentiality.
