Job Summary
Implement daily care plans for agency clients, including hands-on personal care such as bathing, dressing, feeding, and toileting, alongside companion and homemaker services like meal preparation, light housekeeping, shopping, and transportation. Notify the Care Manager of client concerns, changes in condition, or emergencies, and adhere to infection control practices and company policies. Clock in and out per ClearCare Telephony Instructions, updating task status and submitting time logs for non-visit hours as required. Maintain reliable transportation and report to work on time, ready to perform transfers and ambulation using proper safety techniques.
Required Qualifications
- 1,000 hours of long-term care with geriatric or chronically ill persons
- Proof of current TB testing
- Must be physically able to perform the duties of this position
- Must have available reliable transportation
- Must possess excellent communication skills
- Be mature

- Be punctual
- Be reliable
- Pass a criminal background check (national and any required by state)
- Provide verifiable professional and personal references
- Must be able to reposition and transfer clients
- Must be able to sustain weight of clients using proper transfer safety techniques and equipment
- Must be able to travel to client's residence using a reliable means of transportation
Desired Qualifications
