Job Summary
Provide personal care and homemaking services to clients in their homes under the supervision of a Registered Nurse, including shopping, meal preparation, and assisting with activities of daily living. Follow the individualized plan of care to support clients' independence, promote mental engagement, and maintain a safe environment while reporting any condition changes immediately. Perform routine housekeeping tasks, assist with ambulation, and collaborate with therapy personnel and community agencies as directed. Complete weekly activity reports and attend required in-service training. Must be able to drive, lift up to 30 pounds, and possess current CHHA certification, CPR certification, and a valid criminal background check.
Required Qualifications
- Current, valid CHHA certification in good standing issued by the New Jersey Board of Nursing
- Current CPR certification
- Must pass a criminal background check
- Must be able to drive and access clients' homes, which may not be wheelchair accessible
- Must have sufficient hearing, vision, and physical dexterity to assess client condition and provide care
- Must be able to communicate clearly in English, both verbally and in writing
- Must be free from health problems that may be injurious to clients, self, or co-workers, and must provide appropriate documentation
- Must understand and respect client rights, including ethics and confidentiality of care
- Must have sufficient hearing, vision, and physical dexterity to assess client condition and provide care
- Must be able to lift/carry up to 30 pounds
- Must be able to bend, stoop, reach, and move client weight up to 250 pounds
- Must have a Social Security Card
- Must have a Driver's License or State ID
- Must have a Health Physical (within the last year)
- Must have a 2 Step PPD or Quantiferon Gold TB Test
- Must have a Resume
- Must have CHHA Certification
- Must have Rubella and Rubeola immunity with titer or MMR date
- Must have an Auto Insurance Card
- Must have a Federal Fingerprint & Background check
- Must have HHA, CNA, PCA Certification
