Job Summary
Conduct in-person urgent care visits and facilitate telehealth consults for frail, poly-chronic elderly patients in the Rochester/Gates area. Perform home safety inspections, check vital signs including blood pressure and blood sugar, and document comprehensive patient records. Collaborate with interdisciplinary teams to address HEDIS gaps, manage palliative care interventions, and coordinate transitions of care. Attend Interdisciplinary Team meetings, complete daily administrative tasks, and build relationships with local community providers. Rotate through weekend coverage as required while maintaining active state licensure and adhering to regulatory standards.
Required Qualifications
- Master's Degree, Nursing with focus on Family Nurse Practitioner, Adult Nurse Practitioner, Geriatric Nurse Practitioner or Adult-Geriatric Nurse Practitioner from an accredited, Nurse Practitioner program
- Active, unencumbered Nurse Practitioner license in the state of job posting and must reside in state
- Current board certification in your field with either ANCC (American Nurses Credentialing Center) or AANP (American Association of Nurse Practitioners)
- BLS certified and an active NPI# as an NP
- Active, unencumbered DEA licensure or eligibility to obtain
- Valid driver's license with an acceptable driving record
- Proof of an active auto insurance policy
- Ability to conduct in-home setting visits and have previous experience on a telehealth platform
- Demonstrated role proficiency with Microsoft Office; Excel, Word, G-suite, etc.
- Proficient with Microsoft 365 software suite, Electronic Health Records, Scheduling Software, and Ring Central applications
- Role model in courageous teamwork, integrity, ethics, and productivity
- Excellent verbal, written, presentation, and interpersonal communication skills
- Strong communication skills, with the ability to effectively interact with patients, families, and remote healthcare professionals
- Ability to work independently and make sound decisions while adhering to established protocols and guidelines
- Excellent organizational and time management skills to handle administrative tasks and facilitate telehealth consultations
- Experience working with culturally diverse and underserved populations
- Experience in diagnosing and treatment of ongoing, chronic and urgent medical/psychological conditions of older adults
- Experience in management of primary care, geriatrics, internal medicine, palliative care
- Experience in palliative care, and end-of-life communication
- Ability to lift up to 20lbs
- Moving, lifting or transferring patients may involve lifting up to 50lbs as well as assisting with weights of more than 100lbs
- Ability to stand for extended periods
- Fine motor skills, and Visual acuity
- Travel may be required up to 100% locally to deliver patient care
Desired Qualifications
- 3-5 years Nurse Practitioner experience
- Previous experience in value-based care, home care, primary care, hospice, palliative care, or geriatrics
- Other complex patient health care experience
- Experience with telehealth care delivery
- Home care experience
- Previous experience in hospice, palliative care, or geriatrics
- Other complex patient health care experience will be considered
- Telehealth experience preferred
