Job Summary
Provide primary and urgent health care via telephone and televideo to patients receiving home and community-based services through state Medicaid programs and dual eligible members. Develop and implement clinical plans of care for adults with chronic and complex conditions, including co-morbid medical and mental health diagnoses. Gather history, perform physical exams, and prescribe medication as permitted by state authority while maintaining contact with interdisciplinary teams and other providers to coordinate optimal care. Educate patients and families on medication usage, illness progression, and crisis prevention, and maintain medical documentation consistent with state regulations. Work Monday through Friday, 8:00 am to 5:00 pm CST with rotating on-call duties, including required in-person training sessions.
Required Qualifications
- MS in Nursing
- active, national NP certification
- valid, current, active and unrestricted Family or Adult Nurse Practitioner (NP) license in the state of Texas, Arizona, Kansas, Iowa
- valid, current, active, RN Compact license
- 2+ years of experience in managing complex care cases
- Experience working with Electronic Medical Records (EMR)
- licensed in Texas, Arizona, Kansas, or Iowa
Desired Qualifications
- Possession of DEA registration or eligibility
- Experience in managing complex care cases for developmental disabilities and chronically ill patients
