Care Transitions Specialist - Registered Nurse OR Social Worker

Build relationships with hospitals, assisted living facilities, and skilled nursing facilities to identify patients who may benefit from hospice services. Coordinate care plans with patients, families, nurses, physicians, and discharge planners while educating referral partners about hospice options. Support timely admissions and smooth transitions of care, collaborating with the team to improve patient outcomes and track referral activity, goals, and outcomes. Stay current on healthcare trends impacting patient care. This full-time role serves the Keene, Peterborough, and Charlestown, NH communities and requires a Bachelor's degree in Nursing or Social Work with at least three years of relevant experience.
