Care Transitions Nurse - WEEKENDS

Transitional care management nurse role supporting patients discharged from the ED; fully remote, weekends, part-time (20 hours/week) with 8:00 AM–6:30 PM schedule and holiday coverage; requires Bachelor’s Degree in Nursing and RN license in the practice state; typically 2 years of nursing experience in acute care or community settings with emphasis in palliative or senior services; strong written/verbal communication, organizational/analytical/problem-solving skills, ability to educate clinical staff and the community, ability to collaborate with physicians in a multidisciplinary team; may involve home visits using a motorized vehicle; role focuses on discharge coordination and linkages to internal/external resources within 30/90-day programs for Medicare/ACO populations.
