Job Summary
Navigate post-acute care transitions for patients by integrating evidence-based clinical guidelines and protocols into patient-centered transition plans. Represent area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians, and physician groups to drive referral source satisfaction. Maintain a valid state driver's license and dependable transportation to safely drive an automobile in all weather conditions while operating a mobile device. Requires a graduate of an approved nursing, therapy, or social work program with two years of field experience and knowledge of Medicare-certified home health regulations.
Required Qualifications
- Must be a graduate of an approved school of nursing, therapy or social work
- Must be licensed in the state where they currently practice
- Must have two years' demonstrated field experience
- Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice
- Must possess a valid state driver license
- Must maintain automobile liability insurance as required by law
- Must maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
Desired Qualifications
- A registered nurse or physical therapist
- Three years of field experience
- Previous experience in home health or healthcare sales
