Job Summary
The Care Transition Nurse (RN) is responsible for coordinating and managing patient care transitions across healthcare settings, focusing on improving continuity of care and reducing hospital readmissions. Key responsibilities include conducting comprehensive patient assessments, providing education to patients and caregivers about disease management and medications, performing medication reconciliation, scheduling follow-up appointments, and collaborating with healthcare providers. The role requires an active RN license in Connecticut, clinical nursing experience, and a degree in Nursing, along with preferred qualifications such as a BSN and relevant certifications.
Required Qualifications
- Active Registered Nurse (RN) license in Connecticut
- Associate or Bachelor’s degree in Nursing
- 3+ years clinical nursing experience (hospital, case management, discharge planning, or care coordination)
Desired Qualifications
- BSN
- Certification such as Certified Case Manager (CCM) or Accredited Case Manager (ACM)
- Experience with population health or value-based care programs
