Care Transition Nurse - LPN (PRN)

Job Summary
Transitional care nurse collaborates with the multidisciplinary team to ensure an effective discharge across settings including home health, nursing home, hospice care and/or family caregiver; provides continued education post-discharge to patient/family; collects and maintains data for dashboards to improve outcomes; works with RN coordinator to review findings relative to the plan of care; completes NICHE GRN education and participates in geriatric nursing education; maintains professional standards and values of respect, honesty, care, and dignity for the patient and team.
Required Qualifications
- 3 years clinical experience
- Graduated from a Practical Nursing School program
- Current Louisiana LPN license CPR Certification
- Proficient in English, verbal and written communication and computer skills
