Job Summary
Care Transition Coordinator – Hospice, field-based role focused on connecting patients, families, referral sources, and clinical teams to ensure seamless care transitions. Combines healthcare sales, care coordination, and partnership development to support patient outcomes and organizational growth. Responsibilities include liaison between referral sources and clinical teams, executing structured sales calls, building referral partnerships, growing market share, documenting activity in CRM/EMR systems, and supporting timely patient admissions and transitions to home health or hospice services. Requires a Bachelor’s degree (or equivalent experience), at least 2 years of healthcare or related sales experience, strong communication and relationship skills, ability to multi-task in a field-based setting, proficiency with CRM, and a valid driver’s license with reliable transportation. Travel within assigned territory and collaboration with hospitals, case managers, physicians, and community stakeholders are expected.
Required Qualifications
- Bachelor’s degree in Healthcare, Business, Marketing, or related field preferred (or equivalent experience)
- Minimum of two (2) years of healthcare or related sales experience
- Strong communication, relationship-building, and influencing skills
- Ability to manage multiple priorities in a fast-paced, field-based environment
- Proficiency with Microsoft Office and CRM systems
- Demonstrated ability to work independently and drive results
- Valid driver’s license and reliable transportation
