Job Summary
Care Transition Coordinator responsible for referral development, structured sales execution, and coordinated care transitions for hospice services. Serves as liaison between referral sources, patients, families, and clinical teams; builds and maintains relationships with hospitals, physicians, case managers, and community stakeholders; utilizes CRM and EMR systems to track activity and drive referral growth across a defined Mississippi territory (Amory, Houston, Okolona, Starkville, Corinth). Requires 2+ years in healthcare or related sales and a Bachelor’s degree (or equivalent experience); field-based role with regular travel and strong communication, organization, and independent-work capabilities.
Required Qualifications
- Bachelor’s degree in Healthcare, Business, Marketing, or related field preferred (or equivalent experience)
- Minimum of two (2) years of healthcare or related sales experience
- Strong communication, relationship-building, and influencing skills
- Ability to manage multiple priorities in a fast-paced, field-based environment
- Proficiency with Microsoft Office and CRM systems
- Demonstrated ability to work independently and drive results
- Valid driver’s license and reliable transportation
