Care Transition Coordinator

Job Summary
Evaluate patient eligibility and physician orders for home care admission, then conduct face-to-face transitions to educate patients and identify primary care providers. Coordinate transfer orders, ancillary services like DME and infusions, and resumption of care upon facility discharge while documenting all encounters in Home Care Home Base. Manage sales administration duties including expense entry, payroll timesheets, and attendance at weekly meetings to support market growth and financial stewardship. Act as a liaison between the agency, referral sources, and healthcare providers to ensure timely communication and compliance with Right of Choice guidelines and Medicare protocols.
Required Qualifications
- Minimum of one (1) year of experience in home health or hospital-based case management
- Current and active licensure in the state of practice as a Registered Nurse (RN), Licensed Practical Nurse (LPN), Social Worker (SW), or Physical Therapist (PT)
- Must possess a valid driver's license, reliable transportation, and current auto insurance
- Demonstrated understanding of home health eligibility criteria and Medicare/insurance coverage guidelines
Desired Qualifications
- One (1) to three (3) years of experience in medical marketing or healthcare business development
- Respiratory Therapist (RT) certification and/or completion of a technical clinical program demonstrating strong clinical knowledge
