Care Team LPN (Pediatrics)

Job Summary
Coordinate and facilitate structured communication with care team members and providers regarding patient care, pre-visit planning, and documentation of health concerns. Administer medications and immunizations, draw blood, and perform routine clinical procedures such as EKGs and wound care while assisting clinicians with exams and administrative paperwork. Manage the care team's schedule, including work-ins and patient flow, and maintain supply inventories, sterilize instruments, and monitor quality control logs. Collect patient histories, conduct chart reviews, and utilize electronic health records for order entry and documentation updates. Communicate frequently with clinicians and lead huddles to address operational requirements.
Required Qualifications
- High school diploma or equivalent
- completion of accredited Licensed Practical Nursing course
- Minimum of 2 years' experience as a licensed LPN in Health Care, Managed Care, physician practice, or clinic setting
- Ability to demonstrate organizational skills and attention to detail
- good working knowledge of electronic medical records
- Must possess and maintain current/valid Florida LPN license
- Current/valid BLS certification
- Ability to deal with patients effectively in upsetting situations
- able to function professionally in emergency situations
- Able to work flexible hours and sites as needed
- Able to work evenings and weekends
- Background Screening Requirement: This position requires background screening through the Florida Care Provider Background Screening Clearinghouse
Desired Qualifications
- Minimum of 2 years' experience as a licensed LPN in Health Care, Managed Care, physician practice, or clinic setting preferred
- Bilingual skills preferred
