Job Summary
Care Team Assistant supports Providers and Medical Assistants by preparing exam rooms, scheduling follow-ups and procedures, handling discharge steps, and assisting with documentation and patient flow. Responsibilities include maintaining clean, stocked exam rooms; assisting with scheduling; managing discharge summaries and next steps; handling sensitive exam requirements; ensuring faxing and printing are complete; and performing related duties. Requires basic computer and customer service skills, ability to work independently, and cultural competency to serve a diverse patient population. Preferred qualifications include an Associate degree or equivalent college coursework, bilingual capabilities, EPIC and Microsoft Office proficiency, and healthcare or customer service experience. This role is based in Chicago, IL with on-site work at Erie Family Health Centers locations.

- High School diploma or equivalent required

- High School diploma or equivalent required
- Associate degree or two years of college a plus
- Six months of healthcare or customer service experience preferred
- Bilingual fluency to assist or work with non-English speaking patients and/or guests preferred
- Intermediate proficiency in Microsoft Office a plus
- Intermediate EPIC skills are a plus
- Basic computer skills
- Customer service skills
- Ability to work independently
- Cultural competency to work with a diverse patient population
