Job Summary
Assess client and family needs using formal and informal evaluations, referral documentation, and family input. Develop and implement individualized service plans focused on safety, stability, and parenting capacity. Complete all required documentation accurately and on time using the electronic health record system. Track and meet required billable service delivery targets. Supervise family visitation and connect families with appropriate community resources. Maintain professional boundaries and confidentiality while adhering to regulatory standards. This role operates within the Family Support Services team, which focuses on prevention, reunification, and care coordination to support safe, nurturing families. The position requires a flexible schedule including evenings and weekends to accommodate family needs and court orders.
Required Qualifications
- Bachelor's degree in social work or relevant Human Service field
- Minimum of one year experience working in Child Welfare
- Minimum of one year of experience required with a related (non-Human Services) degree
- Act 33/34 Clearances
- FBI Clearance
- Valid, current driver's license and personal automobile liability insurance with a minimum of $300,000 combined single limit coverage
- Approval to verify driving history at the time of authorization and annually; approval to verify insurance coverage at the time of authorization and semi-annually
- Reliable automobile with current inspection
- Ability to meet physical and mental requirements of the job
- Flexible schedule required, including evenings and weekends to accommodate family needs and court orders
