Job Summary
Engage patients to identify barriers to care, implement interventions per care plans, and process referrals from multidisciplinary team members accurately and timely. Coordinate home visits, schedule medical appointments, and provide education to patients and families regarding community resources. Serve as a liaison among providers, specialists, and community services to streamline communication and maintain accurate documentation in care management systems. Participate in quality improvement initiatives and attend departmental meetings as required. Work nights and weekends as departmental needs arise.
Required Qualifications
- High School Diploma or equivalency
- Minimum (3) years of experience in ambulatory/care management or acute care setting, home health, or public health
- Must be able and comfortable working in a variety of settings including, but not limited to clinical environments and office spaces
- Must be able to work nights and weekends as departmental needs arise
- May be required to lift or move items up to 25–50 pounds
- Frequent phone-based communication, requiring prolonged periods of speaking, active listening, and headset use throughout the workday
Desired Qualifications
- Associate degree in Human Services or Health Sciences preferred
