Job Summary
Care Specialist responsible for coordinating all aspects of care for health home members with complex medical/psychiatric needs, integrating physical health, mental health, and social services, ensuring timely follow-up, creating individualized care plans with family and providers, arranging priority appointments, monitoring outcomes via electronic records, and supporting transitions of care. Requires collaboration with a multidisciplinary team, strong communication, crisis-intervention skills, data integrity, and flexibility to cover 24/7 needs including holidays; position based in Queens, NY, with in-person work and access to services within the health home network.
Required Qualifications
- Bachelor’s degree in social work, psychology or a related health/human services field with two (2) years of direct work with the target population. OR Degree/certification in Medical and Clinical Assistance or Health professional field.
