Job Summary
Care Specialist in a Centerstone CCBHC setting provides person-centered assistance to clients for life skills and personal goals; coordinates care with a multidisciplinary team; connects clients with community resources; conducts crisis intervention/prevention for clients and families; maintains clinical records and documentation (treatment plans, progress notes) in accordance with CCBHC standards; teaches activities of daily living in group and individual settings; maintains communications with schools and family service providers; demonstrates strong written and verbal communication, time management, and analytic skills; available in a nonprofit behavioral health system with outpatient, telehealth, and inpatient services; employment includes full-time schedule with benefits.
Required Qualifications
- Associate's degree required
- Two years professional experience working directly with youth and families in related setting preferred
- Two years of experience or two years of college coursework may substitute for degree requirement
