Job Summary
Oversee client care planning and coordination by developing and reviewing care plans in collaboration with clients and multidisciplinary teams. Manage budgets, facilitate care activities, and liaise with clinical staff to monitor and respond to the changing needs of complex community clients. Continuously evaluate care plans and services while assisting clients in navigating the care system and linking them to necessary resources. Spend the majority of your day on the road engaging with clients within the Gympie and surrounding areas. Hold a Certificate IV in Health or Community Service, a current Australian driver's licence, and a roadworthy insured vehicle.
Required Qualifications
- Certificate IV in a Health or Community Service discipline
- Current Australian Driver's Licence
- Roadworthy & insured (Third Party Property minimum) vehicle to use for work
- Demonstrated understanding and sound knowledge of client centred, community-based care services delivery
- Demonstrated ability to liaise with the clinical and care team and to provide co-ordinated care and exercise professional judgment within prescribed areas
- Demonstrated experience and knowledge in Community Funding streams and programs
- Excellent communication and clinical problem-solving skills
- Confidence in managing your time effectively, including an ability to work autonomously
