Job Summary
Coordinate and co-design personalised care plans for older adults, manage a caseload of approximately 45 Support at Home funded customers, liaise with customers, families and internal/external clinical teams, monitor budgets and service delivery, advocate for customers to navigate aged care services, deliver high-quality person-centered care in line with organisational policies, and utilise the My Aged Care Portal to request plan reviews and funding as required, while collaborating with Nursing, Allied Health, call center, and scheduling teams to continuously improve outcomes.
Required Qualifications
- Cert IV in Community Services/Ageing or clinical qualification
- Police Check for working in Aged Care less than 12 months old
- Current driver’s license
