Job Summary
Conduct holistic assessments covering physical, psychosocial, cultural, and environmental needs for NDIS participants and iCare NSW clients. Develop, implement, and review individualized care plans aligned with client goals, wellness, and safety. Serve as the primary contact for clients, families, and carers while advocating for their rights and preferences. Execute regular check-ins, progress reviews, and site audits to maintain quality standards. Monitor client notes, incidents, and service trends to identify risks and escalate appropriately. Report accurately to agencies such as the NDIS and iCare NSW in line with program standards. Grow your client portfolio by engaging with clients, representatives, and industry partners. Requires a valid Australian Driver's License and NDIS Worker Screening Check. Based in Sydney with hybrid flexibility.
Required Qualifications
- A qualification in allied health, aged care, disability or a related community field
- Community engagement experience, including networking and stakeholder engagement
- Strong stakeholder management and relationship-building skills
- Excellent verbal, written and presentation communication
- Strong organisation and decision-making skills, especially in fast-paced environments
- A valid Australian Driver Licence
- A genuine commitment to improving the lives of people living with disability
- A valid NDIS Worker Screening Check
