Job Summary
Care Partner providing compassionate memory-care support to seniors: assist with activities of daily living, observe residents and report changes to licensed staff, document incidents accurately, assist with meals and light housekeeping, maintain linens and equipment, and uphold health- and licensure-related requirements. Must have CPR/First Aid and Food Handler credentials, English proficiency, and experience in elder care preferred.
Required Qualifications
- Review and understand all resident individual service plans
- Assist residents with activities of daily living including bathing, dressing, grooming, toileting, eating, transferring, ambulation
- Observe and report changes in physical/mental/emotional status to licensed nurse or supervisor
- Complete incident reports and written/computerized documentation with accuracy
- Assist with resident mealtimes
- Maintain uniforms/clothes and tidy rooms; change linens
- Assist in cleaning equipment (walkers, wheelchairs)
- Must maintain CPR/First Aid and Food Handler’s credentials
- Must meet health-related state licensure requirements
- Must be able to read, write, comprehend English
- Experience in elderly residential or long-term care setting preferred
- Must maintain continuing education credits
