Care Partner - Memory Care (Evening Shift}

Care Partner for Memory Care (Evening Shift) providing compassionate support to senior residents. Responsibilities include taking vital signs, documenting changes, admitting new residents, assisting with activities of daily living (showers, grooming, dressing, toileting), assisting with meals and dining service, transporting laundry, communicating with health staff and families, and maintaining resident safety and dignity. Requires ability to work evenings, holidays, and weekends in rotation; preferred CNA certification and a high school diploma or equivalent. Must demonstrate empathy, strong communication, teamwork, and the ability to follow care plans and company policies; physical ability to assist residents and perform tasks up to lifting limits as needed.
