Care Partner

Care Partner delivers high-quality, person-centred care coordination across multiple programs including the Commonwealth-funded Support at Home services, the HACC PYP Linkages Program, and the National Disability Insurance Scheme (NDIS). This role ensures that care and support services are planned, coordinated, and delivered effectively and within budget. Works with consumers, families and service providers to develop and implement individualized care plans that promote independence, wellbeing, and quality of life. Through comprehensive assessment, planning, and ongoing monitoring, the Care Partner ensures that each consumer receives appropriate, timely, and cost-effective support aligned with their goals and preferences.
