Job Summary
Care Partner (HHA/CNA) - part-time shift Sun/Mon 3p-11p in New York, NY. Provide resident-focused care including assistance with activities of daily living, medication administration, meal service, housekeeping, mobility, and participation in life-enrichment activities. Collaborate with Integrated Care Team, maintain accurate care and medication records, respond to resident needs, and comply with safety and facility protocols. Requires NYS-approved Home Health Aide certification and a high school diploma; CPR/First Aid preferred.
Required Qualifications
- Successful completion of a Home Health Aide Certification Program approved by the New York State Department of Health
- High School diploma or equivalent
